When a death occurs, it can be difficult to think clearly and rationally about what may be involved in making funeral arrangements. In order to assist you, we have provided a list of some of the important information needed when making the arrangements. You will find it helpful to review this information prior to meeting with one of our funeral directors.


Arrangement Appointment Time
Schedule date, time, and location of services 
Develop the ceremonies 
Select appropriate burial or cremation merchandise 
Coordinate with clergy 
Coordinate services with cemetery 
Coordinate with military, fraternal or religious groups for ceremonial rites
Write and submit obituary notice 

Necessary Information


The following information is required, by law, to complete the death certificate:
Full Legal Name 
Date of Birth 
Marital Status and Name of Spouse 
Father's Full Name and Mother's First and Maiden Name 
Legal Residence 
Years of Education 
Social Security Number 
State or Country of Birth 
Last Occupation and Business or Industry of Occupation 

Other helpful information to be brought in when making arrangements:
Military Discharge Papers (Form DD 214) 
List of Surviving Relatives for Paid Newspaper Notice 
Recent Photograph for Hair Styling 
Insurance Papers (if used to pay for the funeral) 
Cemetery Deed 

Clothing


A full complement of clothing is utilized, including undergarments, however shoes are not necessary. For women, a dress or outfit closed at the neckline and long sleeves are desirable. For men, a suit or sport coat is appropriate.


Personal Items


Jewelry, eye glasses and fraternal or religious items (lapel pins, medals, crosses, etc.) may be brought in when the arrangements are made, or prior to the service. On the day of the funeral, we will confirm which items you wish to be left in the casket and which are to be removed and returned to the family.
